The dialogue surrounding clinical practice, policy, program & service needs for the aging population is evolving – this includes the debate over the strength and quality of community based care.

AgePlan's flagship community programs promote a system where each and every individual is perceived as a citizen with rights, choices and options.

Through AgePlan’s Naturally Occurring Living Communities (NOLC) and Community Assisting Neighbors (CAN) programs, AgePlan advocates for the delivery of services and programs to older individuals in the community.

Naturally Occurring Living Communities

  • • Increases independence & personal choice, and decreases isolation & stigmas
  • • Develops an understanding of the needs of persons & their families
  • • Promotes increasing the individual’s knowledge base regarding services & programs, and how to access existing networks and support systems
  • • Encourages dialogue between public & private entities to develop NOLC-based solutions
